Information about appointment requests, accounts and enquiries on this website. Information you provide The forms collect contact and patient details, appointment preferences, a brief concern and consent. Submitted requests are stored for authorised staff review. Account, consent, payment-reference and administrative activity records are also retained. Closing your browser does not delete a submitted record. Use and access Information supports appointment administration, account verification and communication. Access is restricted by responsibility. Internal staff notes are not shown in the patient account. Do not submit full medical reports, passwords, PINs or OTPs in an initial enquiry. Service providers Hosting and enabled email services process information needed to operate the website. Rootopathy does not sell personal information. Sharing with care professionals or authorities is subject to the purpose of the service, consent and applicable obligations. Your information requests Contact support@rootopathy.com to ask about access, correction, consent or deletion. Identity or caregiver authority may need to be checked. Retention can depend on care, administration and record-keeping obligations; deleting browser cookies does not delete server records. Patient and caregiver information If you contact Rootopathy for someone else, identify yourself and your relationship to the patient. Share only information you have permission or appropriate authority to provide. Making a request for a patient does not automatically give you access to their records. For a child or a patient who needs decision-making support, discuss the appropriate parent, guardian or representative arrangements with the team. Consent and communication Read the information beside each form before submitting it. Consent to an appointment request is separate from agreement to a clinical consultation or permission to share records with a caregiver. Tell the team which contact details are safe to use, especially if you share a phone or email account. Contact support if you want to change communication preferences or withdraw consent; the team can explain any effect on the requested service. Keeping information safe Use the record-sharing instructions provided for your appointment. Ordinary email and messaging can expose sensitive information on shared devices or through forwarded messages. Keep passwords and sign-in links private, sign out on shared devices and report unexpected account activity to support. No online service can guarantee complete security. Storage and retention Rootopathy administers its services from India. Hosting and communication arrangements can affect where information is processed; contact support if you need details before sharing records from another country. Information may need to be retained for care, payment reconciliation, consent history or applicable record-keeping obligations. A deletion request does not automatically remove all clinical or administrative records, including retained backups. Contact Rootopathy: support@rootopathy.com. Website & Care Terms

Version 5 · Approved

Practical terms for using the Rootopathy website and arranging care. Website information Information on this website is educational and does not replace individual medical advice, diagnosis or treatment. Browsing, sending a message or making a request does not by itself establish a doctor-patient relationship. Appointment arrangements A stored request is not a confirmed appointment. Review the clinician, consultation format, date, time, timezone, scope and fee before agreeing. Your patient account records offers, payment status and confirmation separately. Your responsibilities Provide accurate information, explain caregiver authority and tell your treating professional about current treatments. Do not change prescribed care solely because of website content. Keep account and verification links private. Fees and external services Review the stated consultation fee and any separate charges before payment. External providers have their own service terms. Payment verification and appointment confirmation are separate steps. Scope of your consultation Rootopathy provides physician-led care coordination. Agree the purpose of your consultation and ask what review, written information or follow-up it includes. Tests, medicines, procedures and consultations with other professionals are separate from the listed consultation fee. A professional profile or an international price does not establish availability for every patient or location. Caregivers and consent A caregiver may help arrange care with the patient’s permission or appropriate authority. Explain who will attend and what information may be shared. The patient can ask questions about a proposed assessment or recommendation before agreeing. Website form consent does not replace the discussion needed for clinical care. Using your account and this website Use your own authorised account and keep sign-in details private. Do not attempt to access another person’s records, impersonate a patient or professional, upload malicious material or interfere with the website. Contact support if you notice an incorrect account detail or an unexpected payment request. Changes and service questions Read the current service information when arranging a new consultation and keep the offer, payment instructions and confirmation you receive. A website update does not itself change an already agreed booking. If written arrangements appear inconsistent, ask the team to clarify them before paying or proceeding. Questions Contact support@rootopathy.com for questions about service arrangements. Rootopathy operates from Greater Noida, Uttar Pradesh, India.
