Patient Rights & Concerns
Approved · Version 4
Clear information helps patients and caregivers make informed choices.
Before agreeing
Ask about the professional, scope, fee, alternatives and limits of the proposed service. Discuss what matters to you and how a caregiver may participate.
Privacy and access
Ask the team about access to or correction of your information. Patient-specific permission is required for caregiver record access.
Raise a concern
Contact support@rootopathy.com with a brief description, relevant date and a safe reply address. Ask for the responsible administrator or clinician to review the concern. Avoid attaching full medical records to an initial message.
Respect and informed choices
You can ask for a clear explanation of the proposed care, who will provide it and the limits of the advice. Tell the clinician if something is unclear or does not match your priorities. Discuss alternatives, ask for time to consider a recommendation or seek another professional opinion.
Caregiver involvement
Tell the team who you want involved in your consultation and what they may discuss or receive. You may ask to speak privately with the clinician. Let the team know if permissions change. A caregiver or representative may need to explain their authority before receiving patient information.
Helping the team review a concern
Include your booking or enquiry reference if available, describe what happened and explain the outcome you are seeking. Keep a copy of relevant correspondence. If the initial reply does not address your concern, ask for further review by the responsible administrator or clinician. Support can explain the next step for your particular concern.
Your part in safe care
Provide accurate information about symptoms, medicines, allergies and previous care. Tell the team about communication needs and changes in your situation. Treat staff and other patients respectfully, keep other people’s information private and raise problems with an agreed care plan rather than making treatment changes based on website content.
Urgent health concerns
Support and complaints channels are not emergency services. Seek immediate local medical help when needed.
